Our goal is improving clinical outcomes and improving patients' lives through procedures such as balloon sinuplasty and other treatments. The result is the reduction of overall healthcare costs and delivery of an exceptional, differentiated patient experience. BreatheAmerica employs a unique clinical and operational model that we believe makes a significant impact on the lives of patients who suffer from these diseases. This model was pioneered at one of the country's leading academic hospitals, and closely follows a set of widely accepted protocols and guidelines that are proven to produce improved results, but unfortunately are not fully utilized in many physician practices due to a variety of struggles intrinsic to the healthcare system. Services Provided: First Nations Community Healthsource's Social Support Services Program's mission is to provide assistance to American Indians residing in Albuquerque whose family income is as or below 80% of the median family income with their housing, legal documents, vocational, financial, food, clothing, rental and other needs. Our goal is to improve the quality of life for American Indians and by assisting them with their basic survival needs. Services Provided: This program provides parenting educational services, child developmental guidance and local community resource information to families with children prenatal to three in the home or alternative locations at the familys request. Prenatal home visits provide information on health, nutrition, preparing for the birth of the child and child development. Information is also provided on attachment and relationship with infants, breastfeeding, impact of alcohol, tobacco and drugs on pregnancy, seatbelt use, birth control after pregnancy, premature labor, oral heath for mother and infant, folic acid, weight control and diabetes. All pregnant women are screened for depression and domestic violence issues. Families of children birth to three receive child developmental screenings, observation of attachment behaviors between infant/toddler and parent/caregiver, administration of a social support index to assist families with identifying informal support networks and administration of the PICCOLO to assist families with establishing positive interactions with their infants or toddlers. Referrals are provided to community agencies/services as necessary.

Services Provided: The UNM Cleft and Craniofacial Team is an interdisciplinary team for the assessment, diagnosis, education, and treatment of children with cleft lip and/or palate, submucous cleft palate, velopharyngeal incompetence/insufficiency, and all related problems. We assess patients with a variety of craniofacial abnormalities and disorders, such as, 22q11.1 deletion syndrome, microtia and hemifacial microsomia, Robin sequence, Stickler syndrome and other disorders. We have a fully staffed team and work with and refer to multiple other specialties within UNM Children's Hospital as appropriate.The Team's goal is to provide coordination and facilitation of care and is not asubstitute for the patient's local care providers or primary care physician.The cleft clinic is held every month and most patients are seen one time per year or more often if needed and will see all or selected team members depending on the diagnosis or specific needs.
