New Mexico’s new Supports Waiver (SW) is a Home and Community Based Services (HCBS) waiver that is an option for individuals who are on the Developmental Disabilities (DD) Waiver Wait List. Supports Waiver services are intended to complement unpaid supports that are provided to individuals by family and others. A Supports Waiver participant will have the choice to access Support Waiver services through Agency Based Services or a Participant Directed Services. Services include assistive technology, behavior support consultation, customized community supports, employment supports, EMOD, personal care, non-medical transportation, respite, and vehicle modifications.

Visions Case Management and Consulting provides a number of services throughout New Mexico, including case management, DD Waiver case management, Supports Waiver and Mi Via Waiver consulting, court visitor, guardianship, and Centennial Care support broker services. Visions Case Management and Consulting has been family-owned and operated since 1994, and their professional case managers, consultants, support brokers, and guardians live and work in the communities they serve.

Me Town provides Mi Via Waiver and Medicaid Self-Directed Community Benefit services.

Los Amigos provides Brain Injury Coordination, Mi Via Waiver, Centennial Self Directed, and Supports Waiver services.

Self-Directed Choices consultants and administration have diverse experience helping to educate, assist, and guide individuals in all areas of New Mexico. They embrace self-direction principles and inclusive communities. Self-Directed Choices promotes the Know Your Rights campaign and provides information to people and their supports. They support persons of all ages. Their consultants are well-versed to serve people who speak Spanish and Navajo.

Care Network is a provider for New Mexico Guardianship Services, Mi Via Consulting, and Brain Injury Services Fund Service Coordination under contracts with the State of New Mexico. The ADRC is here to assist elders, persons with disabilities and caregivers to find services and resources to help them live well and independently. Programs include Information & Assistance (I&A), Long-Term Options Counseling, Benefits Counseling, Healthcare Options, and Centennial Care Medicaid Waiver for Home & Community-Based Services (Our ADRC handles registry for the state Medicaid waiver). ADRC also provides Health Care Fraud Prevention, Prescription Drug Assistance (PDA), Community Information & Educational Presentation and New Mexico Social Services Resource Directory.

The CDD Supports Waiver program provides a Community Support Coordinator (CSC) for Medicaid Support Waiver participants of all ages as they manage their budget and plan for services. Supports Waiver services include assistive technology, customized community supports, employment supports, environmental modifications, homemaker/personal in-home care, non-medical transportation, respite, and vehicle modification. CSCs clearly explain the Support Waiver process, provide guidance as participants develop and implement their plan and budget, teach participants how to use computer systems and complete paperwork, help participants make informed decisions and troubleshoot issues.
