Young Children's Health Center (YCHC) is a community-based pediatric clinic that provides comprehensive health services to families with children from birth to young adulthood residing in the southeast heights of Albuquerque. Staff is bilingual Spanish/English in order to better serve the community. Special components of this clinic include home visitation to families with babies 0-36 months, behavioral health services, case management, and outreach activities to local schools and the community center, which includes gang prevention and intervention and other neighboring activities. Networking and collaboration with other community programs is a priority and strength of YCHC. Children with special healthcare needs and their families are offered the opportunity to participate in a "medical home" model of individualized care. This year we are expanding our services to include specialists in several fields including cardiology, dermatology, gastrointestinal and pulmonary. Patients and families will now have access to these specialized services in their own community. Referral Status: Patients must live in the 87108 or 87123 zip codes. Services Provided: The New Mexico Behavioral Health Institute is the only state owned and operated psychiatric hospital in New Mexico. NMBHI is made up of five clinical divisions serving a range of public needs. Each division is separately licensed and has its own unique admission criteria. The Mos familiar is the inpatient care we offer adult psychiatric patients. We provide psychiatric services on six units, serving approximately 1000 admissions each year. Our adolescent program is dedicated to treating adolescent sex offenders. Our forensic division offers competency evaluation and treatment for adult patients who have allegedly committed a felony. We also provide treatment for adult sex offenders. NMBHI is a major regional licensed long-term nursing care provider for elderly and disabled residents. NMBHI is CMS certified, accredited by The Joint Commission TJC and provides specialty services for dementia care in both elderly and younger residents. Outreach services are gaining a whole new meaning at NMBHI with the advent of telemedicine. With this technology we are able to assess patients who are in other parts of the state or provide consultation to healthcare providers in those regions. In addition, NMBHI offers community based services though a comprehensive community mental health agency that offers many individual programs.

Services Provided: La Fe is a Federally Qualified Community Health Center, whose quality of services has earned its accreditation by the Joint Commission on Accreditation of Healthcare Organizations. Equally important, La Fe has a long-standing history as a strong minority advocacy organization, which earned it the prestigious National Council of La Raza Affiliate of the Year 2000 Award. La Fe has also gained local, state, national, and international recognition for its innovation in non-traditional health promotion/disease prevention initiatives and educational programs that advance holistic wellness among predominately Mexican-American families. In 2001 for example, the Annie E. Casey Foundation selected La Fe as one of its Families Count National Honorees for its work in strengthening families. La Fe's growth has allowed it to evolve from a one-room tenement clinic, to a comprehensive community wellness network that continues to pursue its original mission statement: To improve the quality of life of those we serve by continuously enhancing the health and human services provided.
