The primary mission of the Vet Center is to welcome home military combat Veterans and to help those in need to readjust and successfully integrate into their communities. DD 214 establishes eligibility. Evidence-based PTSD treatment is commonly provided. Victims of military sexual trauma also receive services. Immediate family members of a service member killed while on active duty are eligible for bereavement counseling. Vet Centers also provide assessment and referral services designed to facilitate successful readjustment to civilian life.

Services include: Infant mental health services MiHijito/MiHijita group Play therapy Parenting with Confidence group Couples counseling and family therapy Fatherhood Program Dialectical Behavior Therapy Therapeutic Behavioral Respite Safe Exchange/Supervised Visitation The Conjunto Preschool forms a major element of the agencys integrative early intervention strategy. This innovative preschool model equally blends students ages 3 and 4, one third have developmental disabilities or delays, one third have emotional special needs, and one third are considered as developing normally. The program promotes self-esteem, cooperative behaviors, and the constructive expression of feelings, as well as developmental growth in language, cognition, motor and self-help skills. The Family, Infant, Toddler FIT program provides a continuum of integrated developmental and therapeutic services to families who have concerns about the development of their young child ages birth to 3. FIT is part of a statewide program that provides early intervention services to infants and toddlers who have or are at risk for delays in development. This programs goal is to strengthen the capacity of families to meet the developmental and health-related needs of their infant or toddler. The FIT staff works with children and their families to achieve optimal physical, emotional, social, and cognitive development. Families also are encouraged and mentored by staff to develop healthy, positive, and nurturing relationships with their children. Services include: Evaluation and assessment Developmental consultation and counseling Family education Home visits Community Resource Center of materials on child development Speech, occupational, and/or physical therapy The Fatherhood Program Padres Unidos focuses on helping men become engaged, committed, and responsible fathers to their children. The program welcomes any father of any age and any background with any needs. The program consists of three parts: An education and support group centered on parenting skills, co-parenting communication, child development, discipline techniques, father rights, legal rights and responsibilities, and supporting a healthy parent child relationship Individual, couples, and family therapy Case management services to help fathers with such issues as education enrollment, resume building, job searches, and resource allocation 10/16/2012

Services Provided: Las Cumbres Adult Services division offers a number of integrated programs that provide residential, employment, day habilitation and community membership opportunities to adults with developmental disabilities in Rio Arriba, Los Alamos, and northern Santa Fe Counties. Las Cumbres offers adults with developmental disabilities the opportunity to live as independently as possible while actively participating in community life. Services aim to enhance quality of life through improvements in the following areas: self-sufficiency exposure to a wide variety of life experiences involvement in their community health and safety ability to develop social relationships self-confidence problem-solving and conflict resolution skills Individuals receive staff assistance at whatever level they need and prefer in consonance with their personal needs, desires, and vision for their future. Supported Living offers round-the-clock services enabling adults with developmental disabilities to live in a home setting. Customized In Home Support staff provide comprehensive supports to clients living in their own homes or with family members. Community Integrated Employment provides ongoing services to persons with disabilities who could not otherwise maintain paid employment in regular, integrated work settings in the community. Day Habilitation provides adults with disabilities a safe, supportive environment where they can learn new skills, socialize with peers, and engage in new activities. Customized Community supports engages clients in activities that promote self-determination, increase independence, bring enjoyment, and enhance the persons ability to interact with and contribute to their community.

Services Provided: Volunteer Peer Counselors age 55 and up make supportive weekly home visits to other seniors who reside in Sandoval County. Peer counselors receive training in basic counseling techniques and mental health issues and receive monthly supervision from a licensed social worker. Counselors visit their clients approximately once a week and provide not only emotional support but also referrals to resources. The program director and volunteers also provide educational programs and discussion groups at senior sites around Sandoval County.

Carrie Tingley Hospital is the only pediatric rehabilitation hospital in the state, housing a 24-bed inpatient unit in the Barbara and Bill Richardson Pavilion. We conduct more than 21 specialized clinics ranging in emphasis from brain and spinal cord injuries to development and neurological conditions to juvenile arthritis and clinical genetics. We also provide numerous therapy rooms, Gait labs, wheelchair evaluations, and braces or prosthetics. Children, particularly those with chronic conditions, need specialized healthcare focusing on their unique needs in a child-friendly place. Hospitalized children with chronic and often life-threatening diseases need quick, easy access to surgical and imaging services available at the Pavilion. And, this continuum of care extends to services available at UNM Children's Hospital, UNM Children's Psychiatric Center and UNM Hospital.

Young Children's Health Center (YCHC) is a community-based pediatric clinic that provides comprehensive health services to families with children from birth to young adulthood residing in the southeast heights of Albuquerque. Staff is bilingual Spanish/English in order to better serve the community. Special components of this clinic include home visitation to families with babies 0-36 months, behavioral health services, case management, and outreach activities to local schools and the community center, which includes gang prevention and intervention and other neighboring activities. Networking and collaboration with other community programs is a priority and strength of YCHC. Children with special healthcare needs and their families are offered the opportunity to participate in a "medical home" model of individualized care.This year we are expanding our services to include specialists in several fields including cardiology, dermatology, gastrointestinal and pulmonary. Patients and families will now have access to these specialized services in their own community. Referral Status: Patients must live in the 87108 or 87123 zip codes. Psychiatric Services are by referral only.

Services Provided: The Columbus Ben Archer Health Center BAHC site was established in 1998. Primary care service area for the BAHC in Columbus includes all communities in Luna County. Deming and Columbus are the larger population centers in Luna County. The primary care clinic is located at 626 Taft Street, an 8,000 square foot, modern medical facility that provides medical and dental services.Columbus, New Mexico, which is about 35-40 miles to the south of Deming, is a small rural community of approximately 1,475 residents located on the southern end of Luna County along the U.S./Mexico border. It's sister city, Puerto, Palomas, Chihuahua, Mexico is only three miles south of Columbus and has a larger population estimated at anywhere between 3,000 to 10,000 depending on whose estimates are used. These two isolated communities depend very closely upon one another, with both relying heavily on agriculture as the dominant economic force in the region. Due to their relative isolation and lack of infrastructure, resources are scarce on both sides of the border. Medical and dental services are offered to all individuals regardless of ability to pay. Transportation to medical appointments is available. Prescriptions are couriered to the site through the main Pharmacy in Las Cruces.

Services Provided: Campo's mission is to create lives of warmth, security, freedom, laughter and belonging for adults with developmental disabilities. The agency specializes in support to adults with an additional diagnosis of mental illness, behavioral disorder and/or brain injury, avoiding the need for hospitalization and successfully supporting people in safe and security community living environments. Campo is also a leader in promoting the rights of people with disabilities and their inclusion in the community. Services provided under the DD Waiver include residential and day services, as well as behavior therapy. In addition, Campo is available to provide non-traditional services under other funding sources, including Mi Via and private pay.

Services Provided: Mental health services on the IOP, PHP levels of care. We work with private PPO insurance companies, with an available sliding scale fee. Some of the areas in which we help people include: depression, PTSD, grief and loss, sexual abuse and sexual trauma, self harm, disordered eating, codependency, sexual compulsivity, love/relationship addiction, alcoholism, drug addiction, substance abuse, process addictions, shame and anger issues.Because we offer customized programs, we are designed to meet unique needs. Services Provided: 4 Winds Equestrian Retreat and Wellness Center Provides Comprehensive inpatient and outpatient programs for substance abuse. We have a 28 day intensive inpatient program that is unique. Not only do we provide traditional talk therapy and educational sessions, we harness the power of equine assisted psychotherapy to facilitate the recovery process. Our outpatient programs are a blending of cutting edge conventional and integrative approaches to provide the best quality of outpatient therapy. Inpatient clients must be 18 years or older.

The Enhancement Center provides mental health counseling through Medicaid and many health insurance plans. These services are provided in Albuquerque, Corrales and Rio Rancho in multiple locations throughout the area. The Enhancement Center also provides Behavior Support Consultation through the Developmental Disabilities, Medically Fragile Medicaid Waiver and Mi Via Programs. These services are provided on site at the person's home, day hab and/or work site.
