Services Provided: The rental on all 75 units is income-based, i.e. family pays 30% of their adjusted income, for rent and utilities and there is adjacent parking for all units. These complexes are located next to the Armijo Senior Meal Site and First Choice Medical Complex and within one block of the new Sheriff's Sub-station. Complex One is known as El Centro Familiar Senior Housing Complex, and is a gated community with all 54 units being equipped with fire and medical alarms. Complex Two is known as Seybold Village and contains 21 handicapped accessible units.

Services Provided: RN Case Management Program: The Medically Fragile Case Management Program provides Registered Nurse Case Management/Service Coordination services statewide for children who are Medically fragile and their families via nine satellite offices. The case managers use a family centered approach to care, and through collaborative partnerships act as advocates, resource persons, facilitators, and paperwork coordinators for families. Case managers support parents as team leaders who address the day-to-day care of their child. The services in the community and through the Medically Fragile EPSDT and Medically Fragile Waiver support families in their decision to care for their child who is medically fragile in their home. Christy Barden, Director, 272-2910, cbarden@salud.unm.edu Eligibility: Individuals who are medically fragile must meet the eligibility guidelines and the definition of medically fragility--a long term chronic medical condition that requires skilled care and assessment. They must also be diagnosed with their medically fragile condition prior to the age of 22 years.Intake Procedure: Referral form is faxed to the MFCMP at 505-272-8100. Services Provided: The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service previously known as a UAP. These centers are authorized by the Developmental Disabilities Assistance and Bill of Rights Act to build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families. The Center for Development and Disability (CDD) is one organization in the Association of University Centers on Disabilities Research, Education and Service (AUCD). The network is coordinated by and receives core funding from the Administration on Intellectual and Developmental Disabilities (AIDD). Funding for other programs and projects is obtained through state contracts, federal grants and private funding. There are 67 centers nationwide, including at least one in every US state and territory, which function as a national network of programs.

All apartments have gas ranges, refrigerators and refrigerated air conditioning.The grounds are nicely landscaped with trees, shrubs and grass. A full-time maintenance staff takes care of the apartments and grounds, maintaining a high standard of excellence.
