Services Provided: Advocacy Partners provides services to individuals with disabilities on the DD Waiver. The services we provide include Family Living, Substitute Care, Community Access. Advocacy Partners was started in 2005 by the Romero family. Our commitment and passion rises from the life we have led and the services we have sought for our daughter. We offer caring and professional support and monitoring for the individuals who provide direct residential and substitute care/respite for people who receive the NM DD Waiver. We offer Family Living Services and Substitute Care/Respite Services through natural families or surrogate providers who are trained and receive clearances from the state of NM. We also provide Community Access. Our agency commits to be partners in our clients lives to assist them, with their Interdisciplinary Team to gain a level of independence that is appropriate for them. We intend to offer true advocacy partnership by employing individuals who are trained self-advocates and can serve as mentors and supports for the individuals we serve. We will provide meaningful job opportunities and additional supports for the people you support. We believe that this is an innovative and creative way to serve a broader population.

We also run a 4-star child care center in Valencia County for children ages 2 to 5 years. Adults: We support adults with developmental disabilities in leading an active life. We help them get involved in the community that surrounds them by organizing community activities, personal care services, and family living situations. We also support families caring for adults with developmental disabilities by giving them temporary breaks through our respite services.Seniors: The primary goal of our services for older adults is to help them remain in their homes with a good quality of life, instead of having to go to a nursing home or assisted living facility. Our team helps seniors with the day-to-day activities that they may no longer be able to safely do on their own. In everything we do we seek to help all individuals, regardless of their age, physical capabilities, cultural background, or income level, to live full and productive lives.

Carrie Tingley Hospital is the only pediatric rehabilitation hospital in the state, housing a 24-bed inpatient unit in the Barbara and Bill Richardson Pavilion. We conduct more than 21 specialized clinics ranging in emphasis from brain and spinal cord injuries to development and neurological conditions to juvenile arthritis and clinical genetics. We also provide numerous therapy rooms, Gait labs, wheelchair evaluations, and braces or prosthetics. Children, particularly those with chronic conditions, need specialized healthcare focusing on their unique needs in a child-friendly place. Hospitalized children with chronic and often life-threatening diseases need quick, easy access to surgical and imaging services available at the Pavilion. And, this continuum of care extends to services available at UNM Children's Hospital, UNM Children's Psychiatric Center and UNM Hospital.

Services Provided: Our ServicesFamily Living Service is a community living service offered under the DD Waiver sponsered by the New Mexico Department of Health Developmental Disabilities support Division. The Family Living Program provides support services to individuals with developmental disabilities in a residential setting designed to promote independence and life skills and to nurture stability by providing a "family" setting. Customized In-Home Supports Formerly Independent Living is a program designed for individuals on the DD Waiver who can live in their own home or apartment, or with family or friends. Customized Commmunity Supports Day Program offers unique, creative and engaging services to our individuals. This includes 1:1 Community Activities, 1:3 Community Group Activities, and a Facility Day program which offers an array of exciting daily activities. Supported Employment The chief intent is to provide individuals with developmental disabilities support in obtaining and maintaining employment. This helps to increase, maintain or promote the individuals capacity for independence, self-determination, self-advocacy, productivity, and community integration. ASI Supported Employment includes: Job Development, Job Coaching, and Job Maintenance. Respite and Substitute Care is a relief service for the primary caregivers. Respite hours are spend with a Respite Provider and may be in an individual's home, the Respite Provider's home or out in the community. Respite Providers are screened and trained per state standards regarding needs and interests of the individuals served.

Services Provided: The location at 404 Hunter Street in Espanola offers the following services: The Behavioral Health services at Las Cumbres offer a prevention-based program that promotes healthy attachment and social-emotional development in children prenatal to age 6 and their families. Services include: Infant mental health services MiHijito/MiHijita group Play therapy Parenting with Confidence group Couples counseling and family therapy Fatherhood Program Dialectical Behavior Therapy Therapeutic Behavioral Respite Safe Exchange/Supervised Visitation The Conjunto Preschool forms a major element of the agencys integrative early intervention strategy. This innovative preschool model equally blends students ages 3 and 4, one third have developmental disabilities or delays, one third have emotional special needs, and one third are considered as developing normally. The program promotes self-esteem, cooperative behaviors, and the constructive expression of feelings, as well as developmental growth in language, cognition, motor and self-help skills. The Family, Infant, Toddler FIT program provides a continuum of integrated developmental and therapeutic services to families who have concerns about the development of their young child ages birth to 3. FIT is part of a statewide program that provides early intervention services to infants and toddlers who have or are at risk for delays in development. This programs goal is to strengthen the capacity of families to meet the developmental and health-related needs of their infant or toddler. The FIT staff works with children and their families to achieve optimal physical, emotional, social, and cognitive development. Families also are encouraged and mentored by staff to develop healthy, positive, and nurturing relationships with their children. Services include: Evaluation and assessment Developmental consultation and counseling Family education Home visits Community Resource Center of materials on child development Speech, occupational, and/or physical therapy The Fatherhood Program Padres Unidos focuses on helping men become engaged, committed, and responsible fathers to their children. The program welcomes any father of any age and any background with any needs. The program consists of three parts: An education and support group centered on parenting skills, co-parenting communication, child development, discipline techniques, father rights, legal rights and responsibilities, and supporting a healthy parent child relationship Individual, couples, and family therapy Case management services to help fathers with such issues as education enrollment, resume building, job searches, and resource allocation 10/16/2012

Services Provided: FootPrints Home Care is a medically licensed home health agency. Our areas of focus are: Senior Care Services: Our first service line, founded in 2005, provides non-medical in home care to seniors. Medically Fragile Waiver Services: The Medically Fragile Waiver funds in home assistance for children ages 0 to 20 living with chronic physical disabilities that require skilled intervention. Since joining in 2008, FootPrints has become the top nursing provider on the waiver. Developmental Disabilities DD Waiver Services: The DD Medicaid Waiver functions similar to the Medically Fragile Waiver but to individuals over 20 years of age. Adult nursing and customized in-home supports are the primary contribution by FootPrints. Services Provided: Ability First, LLC provides care to individuals who are eligible for DD Waiver, as well as private care. Ability First is committed to providing the highest quality of care to individuals receiving Family Living, In-Home Community Support, Supported Living and Respite Care Services.Living Supports consists of both Supported Living and Family Living Services. This is intended for individuals who require 24 hours of care/supervision, seven days a week to ensure health and safety. These services are intended to increase and promote independence, address assessed needs and identified individual outcomes. These services support individuals to live as independently as possible in the community setting of their choice. Living Support providers take positive steps to protect and promote the dignity, privacy, legal rights, autonomy and individuality of each person who receives services.Supported Living is a living environment in which consists of three or four DD Waiver clients who are cared for by awake staff, 24-hours a day, seven days a week. The idea behind Supported Living is to identify individuals that are compatible with one another enough to live with each other on a daily basis. In Support Living, they can participate within the community as a group or individually with staff accompanied at all times. The Direct Care Staff are paid staff who completed a thorough background check and all trainings required by the State of New Mexico.Family Living Services is an individual living in a home environment with one or two primary caregivers, either natural family members or surrogate family. The idea behind this is to live in a natural family-living environment being less restrictive as opposed to Supported Living. The Family Living Provider is a paid provider who completed a thorough background check and all trainings required by the State of New Mexico.Respite Care Services provide support to an individual and give the primary caregiver time away from their duties. The Respite Provider can also assist the individual in many activities listed below. Respite Services may be delivered in the individuals home, providers home or a community setting of the familys choice. Respite Care Providers also must meet the same background checks and all trainings required by the State of New Mexico.In-Home Community Support is designed specifically to increase and maintain the individuals skills, independence and promote self- advocacy. In- Home Community Support is for individuals who need less than 24-hour staff support per day. These individuals will not have overnight staff and will be scheduled with staff support anywhere from five to 25 hours of assistance per week.Customized Employment ServicesOur Services include:Assistance with money managment.Meal planning and preparation.Routine household care.Community integration, access and utilization.Promote, develop and maintain social, spiritual and individual relationships.Implement behavior support and crisis plans.Develop and maintain recreation, leisure and exercise activities.Health maintenance and monitoring.Transportation of individuals to medical and dental appointments, day services, leisure activities and recreational services.Personal Care.Monitor medical and pharmaceutical needs.Provide assistance with self-administeration of medication including the use of medication aides.Nutritional counseling.Assistance to individual who require a wheelchair for mobility and need physical assistance for bathing, dressing and transfers.Implement activities in support of therapy plans.Nursing Services.Home Based Provider agency will provide respite services for direct care providers upon request.
