Back in Use specializes in acquiring, inspecting, sanitizing and distributing used durable medical equipment. AT devices include manual wheelchairs, power wheelchairs, scooters, canes, crutches, walkers, shower chairs, toilet seats, Hoyer lifts, hospital beds, communication devices, recreational devices, visual equipment, and hearing equipment. Through the website www.backinuse.com individuals request usable equipment and those with equipment can donate. The cost of necessities like wheelchairs, are out of reach for many people, and equipment often serves its purpose for only a short time. As a persons condition worsens, expensive items gather dust. Back in Use offers a simple solution. We accept donations of computers, shower chairs, and walkers and give them to people free of charge. Our team is experienced in supporting people with disabilities and we are Microsoft Registered for refurbishing computer. Services Provided: Provides grants of up to $250 to fund Assistive Technology that supports participation in functional life activities. Any New Mexican with a developmental disability may apply for a grant that meets the criteria for functional participation. The yearly funding cycle starts July 1 and ends June 30 of the next year. Services Provided: AAC Loan Bank offers SLPs Speech Language Pathologists the opportunity to borrow one or two devices, for 30 days, free of charge. These devices may be used during evaluations to assess for the appropriateness of features that match those of devices currently available from commercial vendors. Language encoding systems, voice output features and physical access to the device may be assessed with available devices. After this evaluation, a request may be made for the individual's insurance plan to pay for rental of the specific device recommended. Technical assistance is available from the SLP consultant to the Therapy Services Unit to determine which devices may be most appropriate for assessment purposes. Switch Evaluation Kit offers therapists a variety of switches and switch operated devices to assess an appropriate type of switch for an individual's use. This kit is for assessment purposes only and is free of charge. Adaptive Eating Equipment Loan Bank offers therapists an array of adapted utensils, dishes and drinking vessels for assessment purposes. Access to this equipment is free of charge.

Services Provided: Early Childhood Mental Health Services ECMHS at UNM CDD offer relationship-based and family-centered assessment and treatment services for infants and toddlers birth to three and their caregivers/families. We provide assessment and treatment services when families are concerned about a very young child's social and emotional well-being, self-regulation, behavior, and/or when difficulties in childrearing come up for parents/caregivers. Children who have experienced abuse, neglect, disruptive changes in their care-giving environment and/or other potentially traumatic events are an area of specialization for ECMHS clinicians. The Infant Team program at UNM CDD works directly with children birth to three in CYFD-CPS custody, providing a comprehensive assessment of these children and their relationships with primary caregivers. This program provides some case management services, court reporting, and treatment for children and families. Referrals must be made through CYFD or with CYFD caseworker's approval. ECMH Services are provided by licensed social workers, psychologists, counselors, and advanced psychology trainees with specialized training in infant and early childhood mental health approaches. We offer an extended assessment process to be sure we understand family concerns fully before developing a plan for intervention designed to bring relief to both children and their caregivers/families. Parents, other caregivers and family members are our partners in this process and are always actively involved in all aspects of the assessment and treatment process. Services are primarily home-based, or can take place in our offices.

Services Provided: RN Case Management Program: The Medically Fragile Case Management Program provides Registered Nurse Case Management/Service Coordination services statewide for children who are Medically fragile and their families via nine satellite offices. The case managers use a family centered approach to care, and through collaborative partnerships act as advocates, resource persons, facilitators, and paperwork coordinators for families. Case managers support parents as team leaders who address the day-to-day care of their child. The services in the community and through the Medically Fragile EPSDT and Medically Fragile Waiver support families in their decision to care for their child who is medically fragile in their home. Services Provided: The Neurocognitive Assessment Clinic provides comprehensive neuropsychological evaluations for children who have been identified as having a known genetic syndrome with cognitive implications, who have been exposed to alcohol prenatally, or who are Native American children identified as needing a comprehensive neuropsychological evaluation. Additionally, diagnostic evaluations are performed by a multi-disciplinary team to assess children with prenatal alcohol exposure for fetal alcohol spectrum disorder. Clinics are conducted at the Center for Development and Disability in Albuquerque as well as at outreach clinics throughout the state. The diagnostic team includes a pediatrician, a clinical psychologist and a neuropsychologist.

Services Provided: The Autism Programs Parent Home Training PHT program is a no-cost, short-term educational program funded by the NM Department of Health. Families learn to help their child develop and gain new skills through individualized in-home consultation. The PHT approach is guided by the belief that parents are the best and most natural teachers of their young children. Working within the strengths of the current parent-child relationship, PHT consultants help caregivers integrate research-based techniques into daily interactions with their children. Thus, PHT is not a direct therapy program and instead a program to teach families skills for promoting development long after the consultant has left the home.

Services Provided: Partners for Employment is a collaborative approach to increasing employment outcomes for individuals with intellectual or developmental disabilities. The initial partners are the University of New Mexico, the Department of Health Developmental Disabilities Support Division and the Division of Vocational Rehabilitation. The primary goal for Partners for Employment is to build capacity within the state of New Mexico among state agencies personnel, service providers, family members, self-advocates, employers and other supports.Partners for Employment provides training, technical assistance, resource/ information and research on employment supports including provider agencies, state agency personnel, family members, individuals with disabilities, educators and other support systems. Partners for employment offers a variety of training opportunity across the state with national and local leaders in Best practices in employment services. Partners for Employment provides technical assistance to teams providing services utilizing a Project Search model and license. Partners for Employment provides targeted technical assistance to selected teams and individuals referred through DDSD for customized employment. Partners for Employment provide information and resources through tip sheets and video shorts on the information resource network page at CDD. Additionally Partners for Employment is faciliating every other month SELL Supported Employment Local Leaders meeting in each region of the state. Partners for employment hosts a school to work transitiona alliance meeting on a quarterly basis to discuss and make recommendations for improvement in school to work transition. One of the Partners in Partners for Employment is NM Anderson School of Business which is conducting a research project by surveying businesses across the state.

Services Provided: The mission of Continuum of Care is to improve and sustain the quality of health and healthcare for all people with intellectual and/or developmental disabilities in New Mexico. Goals:We positively impact the health of people with I/DD through advocacy, trainings, clinical services, policy reviews, and policy recommendations. We contribute to improving the quality of healthcare through clinical consultations and teaching of clinical services.We support ongoing improvements in the field by evaluating our efforts, incorporating new findings into our trainings and services, developing new research, and sharing our findings. It is estimated that 32,000 New Mexicans have a developmental disability. The range of these disabilities varies greatly. People with disabilities often encounter more challenges than others in the way health care is delivered to them, how they access it and the level of support services they need to stay healthy within the community.The Continuum of Care project grew out of the idea that health care services should be available and delivered in a comprehensive and coordinated manner from infancy to adulthood, assuring people with disabilities access to the full array of health services from a multidisciplinary approach. We promote health care professionals becoming more knowledgeable and competent in dealing with developmental disabilities and in delivering services in a culturally sensitive way that respects individuals preferences, honors personal values and promotes quality of life.

Services Provided: A Medicaid Waiver allows a state, such as New Mexico, to "waive" certain regulations governing certain types of Medicaid services, Medicaid provider requirements and eligibility requirements. A Medicaid Waiver is specifically designed to provide services reimbursable under Medicaid to a targeted population, who in the absence of the Waiver would require services in an institution. Services Provided: This program provides parenting educational services, child developmental guidance and local community resource information to families with children prenatal to three in the home or alternative locations at the familys request. Prenatal home visits provide information on health, nutrition, preparing for the birth of the child and child development. Information is also provided on attachment and relationship with infants, breastfeeding, impact of alcohol, tobacco and drugs on pregnancy, seatbelt use, birth control after pregnancy, premature labor, oral heath for mother and infant, folic acid, weight control and diabetes. All pregnant women are screened for depression and domestic violence issues. Families of children birth to three receive child developmental screenings, observation of attachment behaviors between infant/toddler and parent/caregiver, administration of a social support index to assist families with identifying informal support networks and administration of the PICCOLO to assist families with establishing positive interactions with their infants or toddlers. Referrals are provided to community agencies/services as necessary.
