Services Provided: RN Case Management Program: The Medically Fragile Case Management Program provides Registered Nurse Case Management/Service Coordination services statewide for children who are Medically fragile and their families via nine satellite offices. The case managers use a family centered approach to care, and through collaborative partnerships act as advocates, resource persons, facilitators, and paperwork coordinators for families. Case managers support parents as team leaders who address the day-to-day care of their child. The services in the community and through the Medically Fragile EPSDT and Medically Fragile Waiver support families in their decision to care for their child who is medically fragile in their home. Services Provided: The mission of Continuum of Care is to improve and sustain the quality of health and healthcare for all people with intellectual and/or developmental disabilities in New Mexico. Goals:We positively impact the health of people with I/DD through advocacy, trainings, clinical services, policy reviews, and policy recommendations. We contribute to improving the quality of healthcare through clinical consultations and teaching of clinical services.We support ongoing improvements in the field by evaluating our efforts, incorporating new findings into our trainings and services, developing new research, and sharing our findings. It is estimated that 32,000 New Mexicans have a developmental disability. The range of these disabilities varies greatly. People with disabilities often encounter more challenges than others in the way health care is delivered to them, how they access it and the level of support services they need to stay healthy within the community.The Continuum of Care project grew out of the idea that health care services should be available and delivered in a comprehensive and coordinated manner from infancy to adulthood, assuring people with disabilities access to the full array of health services from a multidisciplinary approach. We promote health care professionals becoming more knowledgeable and competent in dealing with developmental disabilities and in delivering services in a culturally sensitive way that respects individuals preferences, honors personal values and promotes quality of life.

Services Provided: The Brain Injury Association of New Mexico is not a direct provider of services to persons with brain injury. As a private non-profit agency we do offer information, contacts and indirect assistance for individuals living with brain injury, their families, and others looking for information, support and referral to direct services. Services include specialized TBI Case Management to assist individuals and family members in accessing necessary services. The TBI Program offers specialized Life Skills training designed to assist consumers in learning or relearning skills such as money management, cooking, or anger management. The Crisis Interim Fund which serves as a payer of last resort and provides funding for needed services and items. The application for the New Mexico TBI Program from their website listed above. All of these programs are funded by the Traumatic Brain Injury Services Fund.

The department responds to over 12,500 calls for assistance annually with six engine companies, two truck companies, a battalion chief, and several support vehicles. Services Provided: The Branigan Cultural Center has as its focus the history and culture of the region. Built during the Great Depression to serve as the Citys main library, this building was rededicated as a city cultural facility in 1981.The Branigan Cultural Center is comprised of several small galleries and the larger Shannon Room, which is a temporary exhibition gallery and performance hall. Permanent and changing exhibitions and educational programming provide insight into the regions interesting past and lively present.

Services Provided: RSVP offers maximum flexibility and choice to its volunteers. RSVP matches the personal interests and skills of older Americans with opportunities to help solve community problems. RSVP volunteers choose how and where they want to servefrom a few hours to over 40 hours a week. At RSVP it is easy for older adults to find types of volunteer service opportunities that appeal to them. RSVP volunteers provide hundreds of community services. They help children in reading and math, help build houses, help get children immunized, model parenting skills to teen parents, participate in neighborhood watch programs, plan community gardens, deliver meals, prepare income taxes, transport other seniors to medical appointments, visit nursing home residents, help community organizations operate more efficiently and many more.

Services Provided: The Crisis Intervention Response Team, which is part of the assistance program, is on-call 24 hours a day and responds to requests for victim assistance from police. The program provides free services to victims of crime by assisting with the preparation of protection orders, referrals to other agencies, and applications for compensation under the New Mexico Crime Victims Reparation Commission guidelines.The service guides victims through the criminal justice system. These are advocates who are available and free of charge, and have time to listen. Many times just being there for people is what they need most.Victim Assistance tracks 15 major categories of violent crime: voluntary manslaughter, involuntary manslaughter, aggravated assault and battery,negligent use of a deadly weapon, kidnapping, criminal sexual penetrationif response is to the victim's home or the scene of the crime or being reported at a much later date, criminal sexual contact with a minor,physical abuse or neglect of a minor, homicide by vehicular negligence/great bodily harm by a vehicle, abandonment of a child, stalking, harassment, elder abuse, battery against a household member, and aggravated indecent exposure. The program tracks and report crime statistics to the Victim Of Crime Act VOCAgrant administrator at the New Mexico Crime Victims Reparation Commission.
