CARC offers child and adult services including FIT and child development services, adult habilitation, work services, supported employment, nursing, money management, socialization, and opportunities for the Special Olympics. CARC also supports individuals throughout their life span and improves quality of life by providing transportation, therapy pool, on-call services, case management, crisis intervention, and other activities and life skills.

Eligibility:

Residents of New Mexico, anyone under age 21 is eligible if diagnosed with chronic medical conditions or disabilities that require surgical or medical treatment or limit activities. For CMS payment/coverage, there are also financial guidelines.

These conditions are automatically qualifying:

  • Cancer
  • Cardiac Disease
  • Cerebral Palsy
  • Cleft Lip & Palate
  • Cystic Fibrosis
  • Diabetes
  • Epilepsy with Seizure Medications
  • Juvenile Rheumatoid Arthritis
  • Lupus
  • Renal Failure
  • Spina Bifida

Adults are eligible if they have genetic/metabolic or cystic fibrosis who are eligible for lifelong services.

Services Provided:

CMS offers medical services through multidisciplinary pediatric specialty clinics, private providers and public agencies. Diagnostic medical services, information and referral, case management and pre-school developmental evaluations are also available, regardless of income.

Service Include:

  • Medical Management- may include payment for diagnostic services, hospitalizations, physician’s services, and some therapies.
  • Multidisciplinary Pediatric Specialty Clinics- including asthma, cleft palate, genetic, metabolic, nephrology, neurology, and endocrinology clinics
  • Care Coordination
  • Transition Planning for Youth with Special Health Care needs
    Newborn Genetic Screening and Follow-up & Newborn Hearing Screening and Follow-up

Visions Case Management and Consulting provides a number of services throughout New Mexico, including case management, DD Waiver case management, Supports Waiver and Mi Via Waiver consulting, court visitor, guardianship, and Centennial Care support broker services. Services Provided:

Excel's case management services are limited to DD waiver program participants. We contract with the state of New Mexico to provide service coordination through the Developmental Disabilities Medicaid Waiver (DD Waiver). Excel case management aims to support clientele by providing education, advocating, and coordinating services through a variety of programs. We provide case management services to individuals in San Juan County through the Developmental Disabilities Medicaid Waiver, disabled and Elderly Medicaid Waiver, and the Traumatic Brain Injury program.

The UNM CDD Medically Fragile Case Management Program (MFCMP) has offered registered nurse case management/service coordination services statewide for children who are medically fragile and their families since 1985. In New Mexico, medically fragile children are identified as children who have a high level of dependence on medical technology such as ventilators, tracheostomies, oxygen, feeding tubes, dialysis and intravenous medication and require total care for all their daily needs. The case managers at MFCMP use a family-centered approach to care and are advocates, informational sources, facilitators and paperwork coordinators for families. Case managers support parents as team leaders who address the day-to-day care of their child.
