Services Provided: MVCHS provides a full range of outpatient primary medical services including acute and chronic care. Although we are not an urgent care facility, emergency service is available for a wide range of procedures when the need is present. Basic laboratory testing is also available at the clinic and we have a basic pharmacy serving those who are unable to afford their medications. We provide physical examinations for employment and insurance requirements. Specialized services such as OB/GYN and diagnostic x-ray are available through referrals to providers in Las Vegas and other areas. MVCHS offers behavioral health services through group and individual therapy sessions. Our mission is to deliver and coordinate high quality behavioral health services that are readily accessible to residents of Mora County and surrounding areas with the goal of assisting them in attaining optimal functioning in daily life. MVCHS offers a full complement of dental care services provided by a licensed dentist and ancillary dental staff. This includes preventative and restorative dental intervention as well as dental hygiene provided in a state-of-the-art dental suite.

In addition to private pay, we accept Medicaid, Medicare, and private insurance. For the uninsured, a sliding fee scale is available based on income. Services Provided: ARCA offers a comprehensive array of residential, employment and day activities for indviduals with intellectual and developmental disabilities. InterCare homes provide 24-hour supervision for people with development disabilities in a licensed intermediate care facility ICF-MR. InterCare homes have between 4 and ten residents. ARCA also offers services funded through the NM DD Waiver including residential which may include 24 support in a community home or living with a family, customized in-home support periodic support in the person's own home, supported employment, and community inclusion day activity programs. Upon request we will customize supports through private contracts.

Family education offers English as a second language ESL, GED, family literacy, early head start, child care, and vocational training.

Services Provided: The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service previously known as a UAP. These centers are authorized by the Developmental Disabilities Assistance and Bill of Rights Act to build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families. The Center for Development and Disability (CDD) is one organization in the Association of University Centers on Disabilities Research, Education and Service (AUCD). The network is coordinated by and receives core funding from the Administration on Intellectual and Developmental Disabilities (AIDD). Funding for other programs and projects is obtained through state contracts, federal grants and private funding. There are 67 centers nationwide, including at least one in every US state and territory, which function as a national network of programs.

RN Case Management Program: The Medically Fragile Case Management Program provides Registered Nurse Case Management/Service Coordination services statewide for children who are Medically fragile and their families via nine satellite offices. The case managers use a family centered approach to care, and through collaborative partnerships act as advocates, resource persons, facilitators, and paperwork coordinators for families. Case managers support parents as team leaders who address the day-to-day care of their child. The services in the community and through the Medically Fragile EPSDT and Medically Fragile Waiver support families in their decision to care for their child who is medically fragile in their home. Christy Barden, Director, 272-2910, cbarden@salud.unm.edu Eligibility: Individuals who are medically fragile must meet the eligibility guidelines and the definition of medically fragility--a long term chronic medical condition that requires skilled care and assessment. They must also be diagnosed with their medically fragile condition prior to the age of 22 years. Intake Procedure: Referral form is faxed to the MFCMP at 505-272-8100. The Autism and Other Developmental Disabilities Programs at the CDD are a collection of programs that are centered on autism spectrum disorders. We provide clinical services training and technical assistance, as well as family services and supports. The Autism Programs is staffed by individuals with specific expertise in the area of ASD. Our services are statewide.We also provide parents and professionals with knowledge of special needs for people with autism and autistic-like behaviors by providing: diagnostic assessment, family support, advocacy, information and technical assistance. Services Provided: The FOCUS Program, funded through the Department of Health, Family Infant Toddler Program, provides supports and services for families of children aged birth through three years considered to be at medical or biological risk, or environmental risk for developmental delay. Mi Via, which means "my path," "my way" or "my road," is designed to assist you in managing your own services and supports to live your best life. Services Provided: We work to improve the health and well-being of homeless and indigent people in Dona Ana County. We offer healthcare and referral services to individuals and families with Medicaid or to those who do not qualify for other forms of healthcare assistance.
