Adolescent Health Transition Programs

The Continuum of Care project grew out of the idea that health care services should be available and delivered in a comprehensive and coordinated manner from infancy to adulthood, assuring people with disabilities access to the full array of health services from a multidisciplinary approach. 

Services Include:

• Adult Cerebral Palsy Clinic
• Ketogenic Diet Clinic
• Adult Neurology/Vagal Nerve Stimulator in Roswell
• Adult Neurology Clinic in Clovis
• Adult Special Needs Clinic in Albuquerque
• Developmental Disabilities and Mental Illnesses Clinics

Provides special care for children with chronic medical condition.

Conditions Treated:

• Cerebral palsy
• Congenital heart defects
• Genetic syndromes
• Premature babies with lung, eye or feeding issues
• Short bowel syndrome (SBS), or short gut syndrome.

Provides both pediatric inpatient and outpatient services, as well as urgent care visits.

Dedicated to improving health care transition supports for youth moving into the adult health care system, offers education, resources, tools, and state policy advocacy for youth, families, providers, and states. The Six Core Elements of Health Care Transition 2.0 define the basic components of health care transition support.

A national web-based clearinghouse on post-secondary education for individuals with disabilities. Provides national conferences, training sessions, and workshops, develops training modules, publishes resource papers, fact sheets, directories, and website information, and fosters a network of professionals.
