(AIRS RP-5000) These assessments and evaluations may be provided by a variety of professionals, including psychologists (PhD, PsyD), Licensed Clinical Social Workers (LCSW, LICSW), Mental Health Counselors (CMHC, LPC), and marriage and family therapists (LMFT).

Services Provided: MVCHS provides a full range of outpatient primary medical services including acute and chronic care. Although we are not an urgent care facility, emergency service is available for a wide range of procedures when the need is present. Basic laboratory testing is also available at the clinic and we have a basic pharmacy serving those who are unable to afford their medications. We provide physical examinations for employment and insurance requirements. Specialized services such as OB/GYN and diagnostic x-ray are available through referrals to providers in Las Vegas and other areas. MVCHS offers behavioral health services through group and individual therapy sessions. Our mission is to deliver and coordinate high quality behavioral health services that are readily accessible to residents of Mora County and surrounding areas with the goal of assisting them in attaining optimal functioning in daily life. MVCHS offers a full complement of dental care services provided by a licensed dentist and ancillary dental staff. This includes preventative and restorative dental intervention as well as dental hygiene provided in a state-of-the-art dental suite.

In addition to private pay, we accept Medicaid, Medicare, and private insurance. For the uninsured, a sliding fee scale is available based on income. Your emotional wellness is every bit as important as your physical well being. We provide support for a wide range of behavioral health conditions. Services Provided: In the Ramah Chapter of the Navajo Nationas in many parts of Indian Country late detection of breast cancer leads to disproportionally high rates of breast cancer mortality. Ramah Navajos Pine Hill Health Center devised a creative response: it launched a series of "Mammo Days," educational and social outings designed to encourage Navajo women to get regular breast cancer exams. Highly popular, Mammo Days meet a critical need in a culturally sensitive and medically effective way.

Services Provided: ARCA offers a comprehensive array of residential, employment and day activities for indviduals with intellectual and developmental disabilities. InterCare homes provide 24-hour supervision for people with development disabilities in a licensed intermediate care facility ICF-MR. InterCare homes have between 4 and ten residents. ARCA also offers services funded through the NM DD Waiver including residential which may include 24 support in a community home or living with a family, customized in-home support periodic support in the person's own home, supported employment, and community inclusion day activity programs. Upon request we will customize supports through private contracts.

The Arc of New Mexico provides information about developmental disabilities services in New Mexico, including the DD waiver and Mi Via. The Arc can direct individuals to services and programs for adults, families and children, benefits, need for and alternatives to guardianship, trusts and life time future planning, self-advocacy, self-direction and self-determination. The Arc of New Mexico provides guardianship services to persons with developmental disabilities who live in New Mexico and have no family or friends to provide support. A staff of professional guardians is available to assist individuals with developmental disabilities with decision-making.The Arc of New Mexico provides advocacy, information about a wide range of topics related to developmental disabilities. An advocate can be reached by phone or e-mail to answer questions or refer callers to other programs, services and resources that may better meet their needs. The Arc of New Mexico operates a number of programs around the state that address local efforts in various communities to empower individuals and develop communities to better serve individuals with developmental disabilities.

Services include: Infant mental health services MiHijito/MiHijita group Play therapy Parenting with Confidence group Couples counseling and family therapy Fatherhood Program Dialectical Behavior Therapy Therapeutic Behavioral Respite Safe Exchange/Supervised Visitation The Conjunto Preschool forms a major element of the agencys integrative early intervention strategy. This innovative preschool model equally blends students ages 3 and 4, one third have developmental disabilities or delays, one third have emotional special needs, and one third are considered as developing normally. The program promotes self-esteem, cooperative behaviors, and the constructive expression of feelings, as well as developmental growth in language, cognition, motor and self-help skills. The Family, Infant, Toddler FIT program provides a continuum of integrated developmental and therapeutic services to families who have concerns about the development of their young child ages birth to 3. FIT is part of a statewide program that provides early intervention services to infants and toddlers who have or are at risk for delays in development. This programs goal is to strengthen the capacity of families to meet the developmental and health-related needs of their infant or toddler. The FIT staff works with children and their families to achieve optimal physical, emotional, social, and cognitive development. Families also are encouraged and mentored by staff to develop healthy, positive, and nurturing relationships with their children. Services include: Evaluation and assessment Developmental consultation and counseling Family education Home visits Community Resource Center of materials on child development Speech, occupational, and/or physical therapy The Fatherhood Program Padres Unidos focuses on helping men become engaged, committed, and responsible fathers to their children. The program welcomes any father of any age and any background with any needs. The program consists of three parts: An education and support group centered on parenting skills, co-parenting communication, child development, discipline techniques, father rights, legal rights and responsibilities, and supporting a healthy parent child relationship Individual, couples, and family therapy Case management services to help fathers with such issues as education enrollment, resume building, job searches, and resource allocation 10/16/2012

Services Provided: The Community Infant Program CIP provides comprehensive therapeutic home-based and center-based services with a focus on the social emotional and mental health of infants pre-natal to three and their parents and caregivers in Santa Fe County. Licensed Masters-level therapists provide assessment, intervention, education, and treatment through home visits or in groups held on site. Services Provided: Las Cumbres Adult Services division offers a number of integrated programs that provide residential, employment, day habilitation and community membership opportunities to adults with developmental disabilities in Rio Arriba, Los Alamos, and northern Santa Fe Counties. Las Cumbres offers adults with developmental disabilities the opportunity to live as independently as possible while actively participating in community life. Services aim to enhance quality of life through improvements in the following areas: self-sufficiency exposure to a wide variety of life experiences involvement in their community health and safety ability to develop social relationships self-confidence problem-solving and conflict resolution skills Individuals receive staff assistance at whatever level they need and prefer in consonance with their personal needs, desires, and vision for their future. Supported Living offers round-the-clock services enabling adults with developmental disabilities to live in a home setting. Customized In Home Support staff provide comprehensive supports to clients living in their own homes or with family members. Community Integrated Employment provides ongoing services to persons with disabilities who could not otherwise maintain paid employment in regular, integrated work settings in the community. Day Habilitation provides adults with disabilities a safe, supportive environment where they can learn new skills, socialize with peers, and engage in new activities. Customized Community supports engages clients in activities that promote self-determination, increase independence, bring enjoyment, and enhance the persons ability to interact with and contribute to their community.

Services Provided: Volunteer Peer Counselors age 55 and up make supportive weekly home visits to other seniors who reside in Sandoval County. Peer counselors receive training in basic counseling techniques and mental health issues and receive monthly supervision from a licensed social worker. Counselors visit their clients approximately once a week and provide not only emotional support but also referrals to resources. The program director and volunteers also provide educational programs and discussion groups at senior sites around Sandoval County.

Services Provided: The Parent Infant Child Program formerly called the Step Hi program believes that children who are deaf and hard-of-hearing can become skilled community leaders who are self confident and independent. Families receive home visits from a parent adviser who provides information, support and helps families make informed choices regarding communication for their child. The Deaf Mentor Program provides an adult who is deaf or hard-or-hearing to help families understand the issues their child might face. They provide parent support, contacts so other families can talk to each other. They also offer a lending library and a loaner hearing aid bank.

Carrie Tingley Hospital is the only pediatric rehabilitation hospital in the state, housing a 24-bed inpatient unit in the Barbara and Bill Richardson Pavilion. We conduct more than 21 specialized clinics ranging in emphasis from brain and spinal cord injuries to development and neurological conditions to juvenile arthritis and clinical genetics. We also provide numerous therapy rooms, Gait labs, wheelchair evaluations, and braces or prosthetics. Children, particularly those with chronic conditions, need specialized healthcare focusing on their unique needs in a child-friendly place. Hospitalized children with chronic and often life-threatening diseases need quick, easy access to surgical and imaging services available at the Pavilion. And, this continuum of care extends to services available at UNM Children's Hospital, UNM Children's Psychiatric Center and UNM Hospital.
