Services Provided: The Magdalena Municipal School District is a Pre-K through 12th grade school, which is housed on a single campus. MMSD has a multicultural student population of 357. The ethnic structure of the district is approximately 47% Navajo, 29% Hispanic, and 24% Anglo or other. Additionally, in 1988 the district implemented a four-day week schedule to help address and accommodate students who are bussed significant distances. Over half of all MMSD students are bussed, with an average bus ride being 26 miles one way. The longest distance any one student has to travel is 70 miles one way.

AAC Loan Bank offers SLPs Speech Language Pathologists the opportunity to borrow one or two devices, for 30 days, free of charge. These devices may be used during evaluations to assess for the appropriateness of features that match those of devices currently available from commercial vendors. Language encoding systems, voice output features and physical access to the device may be assessed with available devices. After this evaluation, a request may be made for the individual's insurance plan to pay for rental of the specific device recommended. Technical assistance is available from the SLP consultant to the Therapy Services Unit to determine which devices may be most appropriate for assessment purposes. Switch Evaluation Kit offers therapists a variety of switches and switch operated devices to assess an appropriate type of switch for an individual's use. This kit is for assessment purposes only and is free of charge. Adaptive Eating Equipment Loan Bank offers therapists an array of adapted utensils, dishes and drinking vessels for assessment purposes. Access to this equipment is free of charge.

Services Provided: Las Cumbres Adult Services division offers a number of integrated programs that provide residential, employment, day habilitation and community membership opportunities to adults with developmental disabilities in Rio Arriba, Los Alamos, and northern Santa Fe Counties. Las Cumbres offers adults with developmental disabilities the opportunity to live as independently as possible while actively participating in community life. Services aim to enhance quality of life through improvements in the following areas: self-sufficiency exposure to a wide variety of life experiences involvement in their community health and safety ability to develop social relationships self-confidence problem-solving and conflict resolution skills Individuals receive staff assistance at whatever level they need and prefer in consonance with their personal needs, desires, and vision for their future. Supported Living offers round-the-clock services enabling adults with developmental disabilities to live in a home setting. Customized In Home Support staff provide comprehensive supports to clients living in their own homes or with family members. Community Integrated Employment provides ongoing services to persons with disabilities who could not otherwise maintain paid employment in regular, integrated work settings in the community. Day Habilitation provides adults with disabilities a safe, supportive environment where they can learn new skills, socialize with peers, and engage in new activities. Customized Community supports engages clients in activities that promote self-determination, increase independence, bring enjoyment, and enhance the persons ability to interact with and contribute to their community.

A Medicaid Waiver allows a state, such as New Mexico, to "waive" certain regulations governing certain types of Medicaid services, Medicaid provider requirements and eligibility requirements. A Medicaid Waiver is specifically designed to provide services reimbursable under Medicaid to a targeted population, who in the absence of the Waiver would require services in an institution. A combination of medical eligibility and financial eligibility determines an individual's eligibility for Waiver Programs.

Services Provided: The FOCUS Program, funded through the Department of Health, Family Infant Toddler Program, provides supports and services for families of children aged birth through three years considered to be at medical or biological risk, or environmental risk for developmental delay. The program provides a continuum of interdisciplinary services that integrates primary health care with an array of services that include: home visiting, case management/service coordination, developmental assessment and infant mental health intervention, parenting skills and support groups, legal representation from UNM Law Clinic community referrals and support. Serving Bernalillo, Valencia, Torrance and Sandoval Counties Population Served: Infants who have been prenatally exposed to drugs and alcohol.

Services Provided: The Navajo Preparatory School established a School-Based Health Center/Health and Wellness Center SBHC with start up funds from the New Mexico Department of Health/Office of School Health. These may include routine health care, meal preparation and eating assistance, self-help skills assistance, and assistance with developing leisure activities.Recreational Respite offers individuals opportunities to take part in various meaningful leisure activities that are community based such as arts, crafts, games, hiking, music, dance, swimming, gymnastics, spectator sports, learning activities, and community expiration. Our program is a great place to make new friends increase self-esteem and enhance social skills.Life skills Coaching is customized coaching that provides assistance in meeting the unique needs of individuals with TBI. Coaching services include: daily living ADLs, time management, financial organization, dealing with personal relationships, anger management, the use of memory prompts, and how to access social, recreational, education resources and employment.

Services Provided: The majority of our services are provided in Valencia and Socorro Counties. We also provide some adult and senior services in Bernalillo County, children and adult services in Cibola County and McKinley Counties, and adult services in San Juan County. Early Childhood: We provide developmental support and speech and motor therapies in Valencia and Cibola Counties to children ages infant to 3 years with or at risk of developmental disabilities or delay. We help coordinate other services they might need, and offer support to their families so that they can effectively care for the children and meet their needs. We also run a 4-star child care center in Valencia County for children ages 2 to 5 years. Adults: We support adults with developmental disabilities in leading an active life. We help them get involved in the community that surrounds them by organizing community activities, personal care services, and family living situations. We also support families caring for adults with developmental disabilities by giving them temporary breaks through our respite services.Seniors: The primary goal of our services for older adults is to help them remain in their homes with a good quality of life, instead of having to go to a nursing home or assisted living facility. Our team helps seniors with the day-to-day activities that they may no longer be able to safely do on their own. In everything we do we seek to help all individuals, regardless of their age, physical capabilities, cultural background, or income level, to live full and productive lives.

It provides comprehensive behavioral and neurobehavioral health care services. Brookhaven is staffed by an interdisciplinary team of professionals including psychiatrists, physicians, psychologists, nurses, psychotherapists, social workers, speech/language & occupational therapists, nutritionists and mental health technicians.The Neurologic Rehabilitation Institute NRI hospital-based inpatient program is designed to meet the needs of indidviduals suffering from various psychiatric diagnoses as well as the comorbidity of a brain injury or other neurological issues. In addition to this Brookhaven offers transitional and community based services.

Services Provided:

Provides grants of up to $250 to fund Assistive Technology that supports participation in functional life activities. Any New Mexican with a developmental disability may apply for a grant that meets the criteria for functional participation. The yearly funding cycle starts July 1 and ends June 30 of the next year. Funds are distributed each year on a first come, first serve basis until the funds are exhausted.

Services Provided: Abrazos provides a variety of specialized health and developmental services that support families to meet the special needs of their infants, children and adult family members. Our Family Infant Toddler Program is for families whose children ages birth to three have a delay in their development or are at-risk for developmental delay because of a health, medical or other at-risk condition. Services include: Developmental assessment and evaluation and Developmental intervention including Developmental instruction, Occupational therapy, Physical Therapy and Speech Language Therapy. We also provide Family Service Coordination Our Respite Care program provides in home and community based respite to families with a child or adult family member who has a developmental disability. Our Recreational Respite Program for Children with Autism Spectrum Disorder provides a variety of recreational activities year round for children with ASD and includes an After-School program.

Services Provided: A Medicaid Waiver allows a state, such as New Mexico, to "waive" certain regulations governing certain types of Medicaid services, Medicaid provider requirements and eligibility requirements. A Medicaid Waiver is specifically designed to provide services reimbursable under Medicaid to a targeted population, who in the absence of the Waiver would require services in an institution. Services Provided: Confident Parenting Home Visiting provides information, support and encouragement through home-based services for families of infants prenatal to age three. Services are available in both Santa Fe and Rio Arriba counties and Spanish speaking staff are available in both counties. Families receive regular visits in their home by trained Home Visitors who support parents in their role as careivers to promote nurturing parent-child relationships. Home visits connect families to community resources, explore child development and behavior, and support parent-child interactions to promote physical and mental health, safety and child development.

Services Provided: Our center is located among family farms in the Rio Grande River Valley, ten miles north of Las Cruces. Surrounded by pecan orchards, cotton fields, and chile farms, we provide skilled nursing care in a relaxing rural setting. Our therapy gym is one of the best equipped in southern New Mexico and we have full-time, registered therapists from all three disciplines on staff. We also offer dementia care with enhanced activities, therapy interventions, and nursing services focused on the special needs of patients with Alzheimer's disease and related issues.

Bilingual Multicultural Services . Inc, provides individual therapy services SLP, OT, PT as well as workshops for parents, therapists and educators. We serve clients birth-adult from Bernalillo, Valencia, Sandoval, Los Alamos, Santa Fe and Dona Ana Counties including Hatch.We serve many cultures, including all Native American, Hispanic, African, Anglo, Asian and Deaf communities. We offer music-concerts, dances, movement through music. Sign up for art classes, voice-singing. Come and join our fun social groups, which began in February. Every Friday we have a fun social group with music and art. The first Friday is for birth-three, the second Friday is 3-5 yrs and the third Friday is for up to 21 and the 4th Friday is for adults. Make friends bring your friends and family. We provide reading remediation from K to 8th grade. Our staff are trained in multimodality approach to learning and also are trained in Dyslexia. We provide a variety of workshops for parents/significant others. We train future employers and provide support with environmental adaptations in schools, homes and job sites.Some of the trainings are free, some have fees. Contact the agency for information.
