Overview
The Education & Schools section of the Medical Home Portal provides information
about health and learning school issues for children and youth with special health
care needs (CYSHCN). It helps medical providers and school personnel to better work
together to accommodate children with special health care or learning issues and
ensure that they receive appropriate education-related services.
For children who need accommodations or services to effectively
participate in the educational setting, there are generally 2 options to consider,
the Section 504 Plan and the Individualized Education Program (IEP). Your
Individualized Healthcare Plans (IHP) are specific medication and medical
intervention plans that require physician authorization. More information about
these options can be found below, as well as a variety of educational terminology,
background information, useful forms, and resources.
