Services Provided: The Carrie Tingley Hospital Foundation is an independent nonprofit organization responsible for philanthropic giving to enhance the lives of special needs children and their families at Carrie Tingley Hospital in Albuquerque, New Mexico.Carrie Tingley Hospital Foundation offers Arts & Athletics Beyond Limits program AABL including creative exploration in the Keshet Dance Company, Cloud Dancers therapeutic Horsemanship program, power wheelchair soccer and Zia Hot Shots wheelchair basketball team. The AABL Sports Programs are designed for young athletes or potential athletes who have an orthopedic, neurological or neuromuscular impairment such as, but not limited to, SCI, muscular dystrophy, osteogenesis imperfecta, amputation, TBI, rheumatoid arthritis, spina bifida or cerebral palsy. The AABL Program has been in existence since 1998. Scholarships are available. The program consists of:-Muthu Barry Junior Wheelchair Summer Sports Day Camp: Campers ages 8-18 receive structured instruction in archery, tennis, track and field, martial arts, indoor soccer, table-tennis, softball, hand-cycling and weight-lifting. Children with disabilities who do not use wheelchairs may also attend. For youth in power wheelchairs ages 6-20. Teams compete in regional and national competitions. Services Provided: Crossroads for Women provides housing and intensive support services for homeless women with co-occurring mental and addictive disorders working towards self-sufficiency. The agency operates two programs. Maya's Place is a six month congregate living site for homeless women exiting jail or prison. The Crossroads is a two year program utilizing scattered site housing for homeless women cycling between homelessness and incarceration and their children. Our mission is to provide comprehensive, integrated services to support women working to break the cycle of homelessness and incarceration and achieve healthy, stable, and self sufficient lives in the community for themselves and their children.

Services Provided: Dr. Adams is an ophthalmologist who is fellowship trained in Pediatrics. He sees adults, children and is one of the few in town who will treat devleopmentally delayed children and adults. Dr. Adams is also the current president of the NM Academy of Ophthalmology. He does everything from check vision and give diabetic eye exams to surgery of the eye to improve/fix crossed eyes strabismus, "jiggly eye" nystagmus and fix clogged tear ducts nasal lacrimal ducts. Dr. Adams also deals a lot with glaucoma and conditions such as CVI Cortical Visual Impairment. In addition, our offices has an optial shop in it, so as to service it's patients' needs for glasses and contact lenses.

Services Provided: Healing the Children SW has three separate programs which include: HARDSHIP RELIEF PROJECT-For New Mexico Children. Individual cases are referred to HTC by cooperating partners who are healthcare providers. INTERNATIONAL INBOUND CHILDREN PROGRAM-This program brings children to the United States to receive treatment that is unavailable in their homeland. Children are referred by doctors, nurses, embassies, and other organizations. HTC contacts US doctors, nurses, hospitals to donate services and assists with arranging host family and transportation for the child, often using donated frequent flyer miles. SURGICAL & MEDICAL TEAMS-US doctors and nurses travel to Latin American countries to perform on-site surgeries. Hospitals, medical supply and pharmaceutical companies often donate supplies for the trips. HTCSW is an all-volunteer organization, and is one of 12 chapters of National Healing the Children.

In San Juan County, a Local Board governs the EFSP. Services Provided: The majority of our services are provided in Valencia and Socorro Counties. We also provide some adult and senior services in Bernalillo County, children and adult services in Cibola County and McKinley Counties, and adult services in San Juan County. Early Childhood: We provide developmental support and speech and motor therapies in Valencia and Cibola Counties to children ages infant to 3 years with or at risk of developmental disabilities or delay. We help coordinate other services they might need, and offer support to their families so that they can effectively care for the children and meet their needs. We also run a 4-star child care center in Valencia County for children ages 2 to 5 years. Adults: We support adults with developmental disabilities in leading an active life. We help them get involved in the community that surrounds them by organizing community activities, personal care services, and family living situations. We also support families caring for adults with developmental disabilities by giving them temporary breaks through our respite services.Seniors: The primary goal of our services for older adults is to help them remain in their homes with a good quality of life, instead of having to go to a nursing home or assisted living facility. Our team helps seniors with the day-to-day activities that they may no longer be able to safely do on their own. In everything we do we seek to help all individuals, regardless of their age, physical capabilities, cultural background, or income level, to live full and productive lives.

Services Provided: Consumer Direct Personal Care supports a philosophy of individuals being in control of their services and supports.Personal care is available to individuals 21 years of age or older who need personal care assistance in their own home.Receiving home care your way allows you to:Stay in controlRemain in your home and communityMaintain comfort, familiarity and connections with your family and communityDecide how and when to receive care, depending on your needsParticipate in how your services are deliveredSelect and work directly with your caregiverChoose your caregiver who may be a trusted friend, neighbor or relativeThe consumer-directed personal care program, allows qualifying recipients Consumers to choose a provider agency to assist them with hiring, training and scheduling their caregivers. CDPC performs fiscal intermediary functions for the Consumer, including processing time sheets, billing, and caregiver payroll. As a Mi Via Consultant Agency, Consumer Direct provides consultant services to Mi Via Waiver participants. Assistance with development of a plan and budget is provided within rgulartory guidelines while the participant remains in control of which goods and services are purchased and from whom.

We do this by promoting community awareness, and providing educational information to families, educators, and medical professionals and the community about Down Syndrome.
